Falls Prevention
Olmsted Medical Center’s Active Aging Services provides Falls Prevention Consultations. OMC has partnered with Lutheran Social Service (LSS) to provide a multidisciplinary approach to ensure our patients are thoroughly assessed for strength and balance as well as our patients’ homes are assessed for safety. The findings from the multiple screenings are used to build a treatment plan to decrease the risk of our patients having a fall.
Education and resources are provided throughout the consultation, including how to prevent falls and if a fall should occur, how to safely get up after a fall. Active Aging Services will deploy a certified Community Health Worker (CHW) from LSS to complete a home visit prior to the appointment with a medical provider.
During the medical appointment, an Active Aging Services provider will review medications, assess for any medical equipment needs, and work with the patient and his/her support system, if applicable, to build strategies in an effort to prevent falls.
