Active Aging Services

In an effort to become southeastern Minnesota’s primary choice for an age-friendly health system, Olmsted Medical Center’s Active Aging Services (AAS) department was created.

The department provides resources, assists with referrals, provides healthcare and mental healthcare navigation, and ensures education for adults age 55 and better. In addition, the team of experienced staff are available to assist patients, patient family members, caretakers, community partners, and OMC staff with palliative care, memory care, nursing home care, medical cannabis, education and support, and balance-related exercise.

Call, send a portal message, or speak with your healthcare provider about receiving a referral to a licensed professional at OMC’s AAS to receive help with navigating OMC’s primary and specialty services and community resources.

To assist individuals in the OMC service area with identifying, achieving, and sustaining their quality of life indicators to facilitate active aging.

What to expect

You can expect a warm, personal response Monday through Friday from 7:00 AM through 5:00 PM; messages can be left in the off-hours. OMC is focused on providing patient-centered and integrated care to the fastest growing portion of our patient population, ensuring enhanced wellness.

AAS department objective

Experienced care staff will provide information and referrals to ensure assistance is arranged for housing, healthcare, mental healthcare, financial needs, and social connections to meet every individual’s needs and/or goals. AAS strives to encourage preventative health services, improved health and mental health, longevity, and joyful and purposeful independence. Holistic care for aging adults can be provided.

Appointments for residents at The Homestead

Active Aging Services offers appointments for any resident living in the assisted living, memory care, independent living, and High Pointe buildings as well as any staff working at The Homestead. The clinic is open every Thursday from 9:00 AM to 1:00 PM. Services offered include acute (urgent) care, advance care planning, education and support, and primary care.
